Additional Comments:

The MS has been folded into four (since unfolded). There are several small holes where the fold lines join at the centres of the pages, further holes in the upper right-hand quadrants of f.1r and f.2r, and a pin hole in the lower right-hand quadrant of the same. The paper has also been folded to create a left-hand margin on both the recto and verso sides of the page. There is a small tear to the left-hand side of f.1r, and a small stain at the top of the same. The top of f.2 has also been torn, and an attempt has been made to repair the damage with additional paper. There is minor foxing throughout.
